If your form is not filled out 24 hours prior to your appointment, we may not have time to verify insurance benefits on the day of your appointment.

If you have difficulties with online forms, please email info@sodovisioncare.com for a paper copy to print and fill out, or arrive at least 15 minutes early to fill-out in office (else, the examination time will be reduced).
